Yes, you need a router. Windows XP lan is ok for the job.
You may install the EXE in the doctor computer and access it from both computers.
You may review "network applications development" in FiveWin documentation.

if there are only 2 PC´s to connect, you can use a cross-wired cable (direct connection), for more pc´s you need a switch (pc´s are connected to the switch) and for additional internet access you need an internet-router. Normally the router has also switch capabilities, so you need either a switch or a router.
