Hello,

I want to send a email to all my members of the soccerteam.
How should I proceed if I want the folowing

In this mail I would like to send a request to enter there Birthdate and a button to send the data to my already online database with his ID
I have a unique ID that I will send with the mail, but how can I make a form or should I make a form in php online that needs to be
called and send data to ?

Best way to proceed ?
Any small sample's could make the start easier
